Project idea (in English):

Our idea is to establish cooperation in order to achieve better results, accessibility and development of diagnostic and preventive health care infrastructure in the border regions of Ukraine and Poland. The idea is that at the primary level, the community should implement innovative solutions that would provide better preventive health diagnostics, monitor the health of the population and increase the culture of health among residents. We propose to provide communities with opportunities for the work of medical teams and equipping them with appropriate equipment (cars, diagnostic equipment) such brigades will serve a cluster of communities (2-5 neighboring communities) Also in the communities of the participants of the application of IT solutions in primary institutions • to monitor health status and encourage sharing of health data • remote treatment of diseases • easy-to-use digital products designed specifically for seniors • Digital therapeutic tools based on artificial intelligence In general, we aim to establish a dialogue and cooperation between communities and expert medica.
